Chapter 3 of the Faculty Handbook summarizes the leaves of absence for which faculty members may qualify. Specific questions about eligibility and requesting a leave should be directed to your school’s faculty affairs office or the Human Resources Service Center (call (213) 821-8100 or write to email@example.com).
Faculty requesting leaves for family-related reasons should review the section on “New Baby or Adoption” found on the USC Employees Gateway. Faculty members also will need to complete and submit a Faculty Paid Parental Leave Request form following the established procedures of his/her school. To access the Faculty Leave Form, please visit USC Benefits for more information. If the leave request is approved by the relevant dean, the request will be forwarded to the Provost’s Office for a final review and decision. Chapter 9 of the Faculty Handbook provides additional information about Paid Parental Leave.
Information about what to do when requesting a leave for disability reasons can be found on the USC Benefits site.
All other requests for leave should be submitted to the faculty member’s department or division chair (or comparable position) using the Request for Leave form.
